Leadership / management do not listen to feedback given in monthly surveys and fail to discuss these if the results make them look bad

Lack of internal opportunities - these only go to the favourites
Every small mistake is pointed out to you, yet successes are never acknowledged
Complete lack of training for new employees, who are thrown in the deep end, yet management ridicules people for not knowing things they haven't been trained on
Lack of clear communication regarding process changes, which take place constantly
Lack of flexibility and breathing room in
certain roles
Rewards given out to favourites for doing nothing
