executive management micro managed way too many things
executives always pushing unrealistic revenue targets. never made the overall target
executives not listening to customers, nor listening to mid managers/ directors
lots of chiefs for so few workers
engineering innovation was lacking
engineering team not motivated
production team lack of control over process and vendors caused shortages
