I was hired for a Customer Service Representative I position and requested a reasonable accommodation under the American with Disabilities Act (ADA). HR then informs me that they would be pushing out my start date until they review my request and the paperwork signed by my health care provider. I get another call from Sentry just as I finish setting up my equipment saying they will not be moving forward with the offer as they have determined my request is "fraudulent" because they received two different sets of paperwork. I try to explain to the HR representative that it's not fraudulent and this has got to be some sort of mix up. I try to further explain my health care provider updated a few things on the ADA form and the reason they didn't get the updated form is because there's been known issues with outgoing faxes from my health care provider's machine and I'm met with "at this point, we are not interested in continuing with this conversation." I then ask that the updated form that they never received from my health care provider be re-faxed by my PCP. Their office provided me with proof Sentry received the form and then I get an email stating my Payment Election Event was rescinded.
