Lack of leadership from senior management, limited communication among regional management team, poor strategic vision, questionable ethical decisions, reactionary response to market fluctuations (versus being an industry leader), loss of focus on the customer and their needs, unrealistic view of the market and the price it can bear, insufficient time and resources to accomplish goals within reasonable parameters, cursory support of safety and regulatory compliance, ponderously slow in moving forward on any front, and absurd rate of turnover within all levels of the organization.
