A lack of integrity, last in class HR (spends most of their time covering up inappropriate behavior or their poor work), bad management (senior leaders "retire" overnight and are wiped clean from company websites on a regular basis), and unengaged employees who have been trained to keep quiet when millions are wasted on new initiatives or are told to enact plans that obviously have been drafted by folks who don't understand the technology, the particular business, or infrastructure.
