Boys club culture, a lot of unconscious bias and a few outdated attitudes. E.g. people complain about positive discrimination when their are 90% white men in technical roles is a bit bonkers.
Not unusual for women to change departments due to a few misogynistic bad eggs, HR aren't active in supporting issues. Issues generally hit a ceiling and aren't resolved.
