Leadership is weak from the top down and their priority models are completely selfish and/or jaded. None of them have any idea how to make things better and they don't seem to see how what they're doing has created such a corrupted state. Very basic questions of "How do we do this thing?" are not well-defined and people having to make things up as they go along creates stress and tension on everyone.
Even the basic things that are defined are not done so at any sophistication expected of a company of 20+ years because the CEO has no idea how to make a good business.
