Poor C-level Leadership -- it's hard for top leadership to communicate vision.
Poor CS leadership -- they hired some fly-by-night gang of CS operators who had no experience in horizontal/complex/developer platforms or operating at this scale. Didn't understand how to enable CS teams to drive adoption and customer value. Just a bunch of "document writers" with expert in fluffy language. I've heard that this entire gang was eventually fired but the damage has been real -- several good people in Customer org have left. Worse, they replaced this gang with Solution Engineering leaders who have no experience in CS. So some more people left and now DKU is struggling to hold on to their employees and customers.
