What I have experienced:
Workers are not treated equally/fairly - this all depends on your role and who you are. Favouritism.
Toxic from the top down / tone deaf.
Workers are not truly appreciated/respected or supported. Client is always right mentality.
Leadership dangle carrots but don’t deliver.
Lack of centralised databases/process/systems when a big portion of the work is benchmarking and using data to find cost saving opportunities.
Lack of expertise / knowledge / organisation within teams which makes it difficult to deliver quality work to clients.
