They didn't pay their employees nearly enough to live on, despite many having undergraduate and graduate level degrees, they didn't properly value them and refused to hire enough employees to accomplish the necessary tasks, and their system was disorganized and chaotic as a result. Samples were kept for much longer than necessary which created clutter and samples were often discarded accidentally as a result of poor organization. Upper management didn't communicate effectively or listen to employee needs.
