Promoting/hiring wrong people for job advancement. Many never had management training or skill sets. Upper management failed to recognize the issues and believe that the organization is one of the best place to work for. It is not. People with experience/ skill set are over looked and promotes individuals with no background. Then management wonder why the department isn't effective, efficient and meeting the yearly goals.
