The hiring manager and the role senior to them were on completely different wavelengths about what the role entailed. Different descriptions entirely. (Red flag!) There was game-playing with the questions where it seems they were trying to trip me up. (Another red flag, who wants an environment where they purposely try to trip you up?)

Screened by the temp agency. Phone interview with hiring manager. Face-to-face interview with hiring manager. Interest expressed and background check required. Hired by manager, but employed but through the agency for 3 months.
