There is a tremendous lack of focus and agreement happening at higher levels, leading to a lot of unnecessary long-term planning which is inevitably dumped in favour of rushing from one fire to the next. Related to this, leadership is generally a strange mixture of capable individual contributors who have risen to their station and struggle to lead, and those who have been hired to manage but have nothing else to contribute and a vague understanding of technology in general.
